YOU MUST HAVE A PHYSICAL ON FILE IN THE ATHLETIC OFFICE BEFORE TRYING OUT OR PRACTICING FOR YOUR SPORT!

Your Physical MUST be from after the date of April 15 for it to be accepted for the following school year.  For example after April 15, 2022 makes your physical acceptable for the 2022-23 school year.

PLEASE PICK ALL SPORTS THAT YOU MIGHT PARTICIPATE IN THROUGHOUT THE WHOLE SCHOOL YEAR (Fall, Winter, Spring)!  If you don't make a team and will join a team that doesn't cut please pick both the sports.  For example if you don't make a basketball team and you want to wrestle, pick both sports

DO NOT EVER PAY BEFORE MAKING THE TEAM!!  We CANNOT REFUND you if you pay before the team is set and you do not make the team!  DO NOT USE SCHOOL PAY!!!  Once you make the team, you will pay directly through Final Forms or to the athletic department with cash or check.  IF YOU ARE FREE AND REDUCED LUNCH, PLEASE MAKE SURE YOU FILL OUT THE PAPERWORK, SO THAT WE SEE YOU ARE IN FACT FREE AND REDUCED LUNCH.  Prices are reduced by half for free and reduced student athletes and their families.

High School

1st sport of the year = 75 + 5 (final forms fee each year) for a total of $80

Free and reduced 1st sport of the year = 40 + 5 (final forms fee each year) for a total of 45

2nd sport of the year = 75

Free and reduced 2nd sport of the year = 40

3rd sport = FREE!

Middle School

1st sport of the year = 60 + 5 (final forms fee each year) for a total of $65

Free and reduced 1st sport of the year = 30 + 5 (final forms fee each year) for a total of 35

2nd sport of the year = 60

Free and reduced 2nd sport of the year = 30

3rd sport = FREE!

Family cap = $400, $200 for free and reduced

We will see the physical in Final Forms once you have created a final forms account and uploaded the physical/  Please make SURE ALL information is filled out properly before submitting your physical.  

FinalForms allows you to complete and sign athletic participation forms for your students. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
